Job Description

Job Purpose

Responsible for the administration of the kidney disease programme which provides disease specific case management for patients with chronic kidney disease (CKD Stage 1 to 5) and on dialysis.

Duties and Responsibilities

Primary Responsibilities and Duties (80%)

• Assess, plan, implement and evaluate care of patients with renal disease
• Conduct teaching and counseling for patients and their families / carers
• Conduct / assist to conduct specialty nurse-led clinic for patients and their families where needed
• Co-ordinate patients\' access and treatment sequence where required
• Assist with overall coordination of renal replacement therapy
• Assist with placement of patients in dialysis facilities as required
• Deliver patient care (inclusive of education and counseling) according to the established standards of care and practice using patient care plan / clinical pathways
• Contribute to multidisciplinary patient management
• Apply understanding of prevention of medico-legal issues in relation to patient care
• Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services

Secondary Responsibilities and Duties (20%)

• Participate in activities that contribute towards patient care improvement.
• Participate in quality improvement activities
• Take on additional responsibilities or tasks as assigned by the Reporting Officer / HOD

Job Specification/Requirements

Qualification:

• Diploma or equivalent professional qualifications in Nursing, Social Work or Allied Health or/and any clinical disciplines relevant to the job is preferred.

Skills:

• Critical thinker, self-motivated, able to work as part of a multi-disciplined team, and have excellent interpersonal and communication skills is preferred.
• Ability to use local languages and dialects will be an advantage.
• Equipped with basic computer skills in MS World, Excel, and PowerPoint.
• Knowledge in geriatric care, statistics, and community services will be an advantage.
