DESIGNATION : Assistant Care Manager / Care Manager (CREST)

RESPONSIBILITIES
As a Care Manager, you will be responsible for organising and conducting outreach to increase mental health and dementia awareness and promote early identification. You will support clients who are at risk of mental health issues and/or dementia by linking them to the necessary support and resources. You will continue to monitor and support these clients in the community. You will also be involved in data collection and evaluation of the CREST programme. Programme Development and Implementation

  • Provide care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support.
  • Plan and conduct outreach to the community and stakeholders including faith-based, corporate and community organisations to promote mental wellness and mental health awareness.
  • Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.
  • Plan and organise structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
Administration and Staff Development
  • Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance with the CREST programme requirement.
  • Keep and maintain client and programme data and documents, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.
  • Support in the development of CREST service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, service linkage & follow-up).
  • Ensure attendance in training and meetings, where relevant, as part of the staff competency required by the CREST programme.
Other Duties
  • Participate in relevant meetings with key stakeholders including AIC to share key learning points where required.
  • Any other duties, as assigned.


QUALIFICATIONS
  • Bachelor's degree preferably in Social Work/Psychology/Gerontology/Counselling
  • Diploma with social service experience/SSTI Diploma in Social Service/Gerontology
  • Minimum 3 years’ experience in community healthcare settings in Singapore
  • Good knowledge of the geriatrics and community services
  • Proficient in Google Workspace (i.e. Google mail, docs, slides, sheet etc)
  • Good communication skills (oral and written)
  • Passion in community services and service oriented
  • Good judgement and decision making skills
  • Good in problem solving
  • Good record keeping abilities and attention for details
  • Meticulous and independent
