Adopt a case management approach when working with clients ie. network with other agencies/specialists to provide multi-disciplinary assessments and monitoring of clients
Ensure proper documentation of client\'s progress and care plans
Ensure all clients\' records are appropriately kept
In consultation with the Case Manager, provide training and supervision to CAN Carers including recruitment and orientation
Communicate, supervise and support CAN Carers
Monitor home visit schedule of CAN Carers
Participate in outreach activities and events
Collect statistics and data relating to service operations
Assist with administrative duties relating to the Centre\'s operations
Any other duties as assigned by the Center Director/ Social Worker / Case Manager
Qualifications:
At least a Diploma in Social Service, Psychology, Counseling or any other related disciplines
Other Information:
Previous experience in social work or related field or work with older adults in long term care is desirable
Proficient in speaking & writing good English. Proficient in speaking and writing a 2nd language and ability to speak dialects will be an added advantage.
Must have a person-centered approach when assisting seniors
Good written communication skills to document information in a clear, concise and accurate manner
Possess excellent interpersonal skills
Proficient in Microsoft office (Word, PowerPoint, Excel)
