About the Role

• Work closely with the Care Coordinator Team to provide basic care coordination, discharge planning, monitor/evaluate and education for patients.

• Conduct telephone follow up to build rapport, obtain feedback and engage patients and their caregivers.

• Support caregivers in caring for patients.

• Promote healthy lifestyle education, including educating the carers of the patients.

• Participate in community health lifestyle activities.

What We Are Looking For\xef\xbb\xbf

• Candidate must possess at least a Diploma

• Diploma in the following discipline will be helpful, include: Diploma in Health Management & Promotion, Diploma in Community Development and Diploma in Social Science)

• Fresh graduates are welcome to apply

• Good communication and interpersonal skills

• Team player and someone who wants to contribute towards community services

• Proficient in computer skills and Microsoft office

• Able to work independently and with a team

• May need to work on weekend for community activities
