Job Description


Key Responsibilities:

  • Develop and implement community health plan for the precincts within Jurong West, with emphasis on preventive health and social health integration.
  • Identify partners for the development of healthy precincts in Jurong West and collaborate with them to enable successful on-ground activation.
  • Track and review progress on implementation of community health plan.
  • Conduct research on precinct characteristics and resident profile to identify the needs and opportunities within the precinct.
Required Competencies:
  • Analytical abilities to integrate information from different sources.
  • Adept in stakeholder management to foster buy-in and collaboration.
  • Preferably strong in writing and verbal communication abilities.
  • Resourcefulness in problem solving and resilient in overcoming challenges.
  • Preferably knowledge and experience in stakeholder and project management.
  • Preferably knowledge of Singapore healthcare system and/or population health models
  • At least 3 years of relevant working experience.
