Key Responsibilities:

Develop and implement community health plan for the precincts within Jurong West, with emphasis on preventive health and social health integration.
Identify partners for the development of healthy precincts in Jurong West and collaborate with them to enable successful on-ground activation.
Track and review progress on implementation of community health plan.
Conduct research on precinct characteristics and resident profile to identify the needs and opportunities within the precinct.

Required Competencies:

Analytical abilities to integrate information from different sources.
Adept in stakeholder management to foster buy-in and collaboration.
Preferably strong in writing and verbal communication abilities.
Resourcefulness in problem solving and resilient in overcoming challenges.
Preferably knowledge and experience in stakeholder and project management.
Preferably knowledge of Singapore healthcare system and/or population health models
At least 3 years of relevant working experience.
