Work closely with the Management to set direction for and implement capability development initiatives for staff on matters pertaining to care and support, network development and community care.
Develop and review internal and external processes across management and community partners to ensure proper care transition, care integration, coordination etc
Develop and review care and support protocols and practice guides to ensure care processes and standards are established across management i.e. triaging, care assessment, coordination, intervention and monitoring.
Support capability building and professional development efforts to ensure that the management staff is trained to perform those as mentioned earlier.
Support overall network development efforts across government and non-government agencies to support organizational priorities focused on building our community care ecosystem
Support the development of local networking building strategy and engagement plan to support network and community care ecosystem development.
Gather ground feedback from the management and agencies to identify service gaps and drive the development of initiatives and interventions to meet seniors\' needs
Job Requirements:
Degree in any discipline with at least 2 years of working experience preferably with knowledge and familiarity with the healthcare or social sectors
Analytical and organized, able to translate concepts into actionable goals
Confident in working with external stakeholders
Good in communication, both verbal and written
Meticulous, team player, resourceful, and able to multi-task in a fast-paced environment
