• Work closely with the Management to set direction for and implement capability development initiatives for staff on matters pertaining to care and support, network development and community care.

• Develop and review internal and external processes across management and community partners to ensure proper care transition, care integration, coordination etc

• Develop and review care and support protocols and practice guides to ensure care processes and standards are established across management i.e. triaging, care assessment, coordination, intervention and monitoring.

• Support capability building and professional development efforts to ensure that the management staff is trained to perform those as mentioned earlier.

• Support overall network development efforts across government and non-government agencies to support organizational priorities focused on building our community care ecosystem

• Support the development of local networking building strategy and engagement plan to support network and community care ecosystem development.

• Gather ground feedback from the management and agencies to identify service gaps and drive the development of initiatives and interventions to meet seniors\' needs

Job Requirements:

• Degree in any discipline with at least 2 years of working experience preferably with knowledge and familiarity with the healthcare or social sectors

• Analytical and organized, able to translate concepts into actionable goals

• Confident in working with external stakeholders

• Good in communication, both verbal and written

• Meticulous, team player, resourceful, and able to multi-task in a fast-paced environment
