Provide basic monitoring and support services for seniors to maintain overall health status and ensure the seniors\xe2\x80\x99 safety within the Senior Group Home (\xe2\x80\x9cSGH\xe2\x80\x9d)
Educate and assist the seniors and their caregivers to monitor their own health status e.g. blood pressure, glucose monitoring, medication reminders, etc
Provide care coordination service and assist the seniors to access other social and healthcare services to ensure their care needs are adequately addressed e.g. meal services, home care, home help, financial assistance, hospital admissions, medical appointments and case management/counselling service, etc
Conduct periodic review of seniors\xe2\x80\x99 health, social and functional status and to develop and implement care planning for seniors under the assisted living model
Responsible for the overall living environment of SGH, including safety, maintenance and cleanliness
Provide Information and Referral service for enquiries relating to SGH and other case management related services.
Assist in the administrative aspects of the SGH including, but not limited to, service scheduling and coordination, submission of data and indicators as required by the authority
Requirements:
Experience in eldercare or nursing will be advantageous
Proficient in MS office (Word, Excel, PowerPoint) & cloud systems
Good team player and able to work independently
Good communication and interpersonal skills
Able to multi-task and adapt well to changes
Self-motivated and organised
Strong administrative skills
Qualifications:
Diploma, \'A\' or O\xe2\x80\x99 Levels or ITE certificate
