STATEMENT OF PURPOSEThe Assistant Manager, Primary Care Engagement & Care Transformation (ECT), Primary Care Office (PCO) will support the ECT team to develop a network of primary care and community partners, and foster partnerships to build an integrated care ecosystem with the vision of a healthier population in the Central-North region.

  • MAJOR DUTIES AND RESPONSIBILITIES
Primary Care Engagement & Partnership
  • Plan and execute partnership strategies that build the network of primary care and community partners, to provide accessible, appropriate and affordable care in the community;
  • Co-develop the partnership management framework (including operating, outcomes and funding models) by which the network of primary care partners will operate;
  • Foster and maintain relationships with General Practitioners (GPs) to understand their strengths and needs, and collaborate to meet the health and care needs of residents in Central-North Singapore.
  • Connect resources with primary care doctors and work with stakeholders within and beyond National Healthcare Group to build their capacities and capabilities to care for their patients and solve system and process problems
  • Plan and implement collaboration programmes, including shared care programmes, to integrate care seamlessly between primary care and tertiary care to improve patient outcomes;
  • Plan and improve access of services for primary care partners including direct access, shared and community care services;
  • Plan and implement quality improvement framework to support Healthier SG preventive care objectives with primary care partners;
  • Identify opportunities for capability development, facilitate the implementation and use of collaboration platforms for the sharing of learnings, best-practices and training.
  • Financial management:
  • Prepare and analysis of programme budget report
  • Assist the team in ensuring that the department operates with sound and sustainable financial models for all initiatives, services and efforts.
  • Operations management:
  • Trend analysis and benchmarking of key performance indicators for the assigned track.
  • Manage, organize, and coordinate of operational processes of the areas assigned.
  • Service and performance measurement
  • Fulfill MOH/NHG\'s service standards for patient care settings.
  • Coordinate and maintain relevant quality systems
  • Identify and monitor customer service standards and KPIs.
Staff Development
  • Development, supervision and guidance to staff to ensure good leadership, knowledge transfer and team succession planning in the assigned track(s).
  • GENERAL
  • Promote continuous process improvement/ review of the assigned programmes and projects, to meet the organisation\'s objectives and patients\' / community\'s expectations.
  • Responsible for managing, monitoring, and controlling identified risk and including the implementation of mitigation plans.
JOB REQUIREMENTS(A) EDUCATION, TRAINING AND EXPERIENCE
  • A degree in any related course.
  • Minimum 4 years of relevant/ related working experience.
  • Possess good interpersonal and communication skills (written & verbal).
  • Familiarity with the concepts of budgeting, operations and administration of community/ primary care/acute/ intermediate and long term healthcare sector is advantageous.
