Working closely with Senior Activity Centre (Cluster Support) [SAC(CS)], Senior Activity Centre (Rental) [SAC(R)], Caring Assistance from Neighbours (CAN) Carers team as part of the Senior Cluster Network (SCN) to provide holistic support for the seniors;
Work with AIC to ensure that SGH clients are adequately supported with comprehensive social and home care services such as Meals on Wheels, Housekeeping, Personal Hygiene.
Provide basic care coordination service for clients\xe2\x80\x99 healthcare, social and financial needs and match clients to the appropriate services etc. This includes monitoring the clients\xe2\x80\x99 well-being regularly such as gentle reminder to any residents who are going for medical appointment.
Work with the SCN Cluster Operators (COs) to establish formal and informal networks and linkages with external parties such as community-based agencies, grassroots organisations, hospitals, polyclinics and neighbours of seniors to meet the social and care needs of the seniors
Establishing standard operating procedures to ensure that minimum service standards are met.
Participate in case conference with respective Government Agencies / Community Partners / Hospitals involved the residents of SGH.
Perform other organizational duties as assigned by the Centre Director
