Job Description

:
As a Case Associate,, you will be required to provide supportive services to seniors of the Henderson SeniorCitizens Home, Senior Group Home and CCMS clients. The role focuses on (i) maintaining a safe and cleanhome environment for the Henderson Home's residents, (ii) engaging residents and raise their concerns tosocial workers or Head of Home, (iii) partnering colleagues in providing holistic care for Henderson Home's residents.
Care and Support

• Monitoring the well-being of seniors, ensuring their safety, comfort and care needs are met
• Assisting seniors with monitoring their own health e.g. blood pressure and glucose monitoring
• Maintaining a clean and safe environment for residents
• Supporting residents with exercises and medication adherence
• Coordinating cases of seniors with social workers
• Assisting in new resident admissions
• Organising visits for residents medical check-ups and accompanying them to medical appointments
• Maintaining the residents movement logs for essential needs such as doctor appointment
• Facilitating seniors access to essential social and healthcare services, eg meal services, home care, homehelp, hospital admissions, medical appointments and counselling service etc.
• Tracking visitors activity by the visitor log book
• Supporting outreach initiatives at Henderson Home

Emotional and Social Support

• Facilitating group activities to enhance seniors social interaction and mental well-being
• Providing emotional support to seniors facing challenges
• Mediating conflicts between seniors when necessary

Administrative Functions

• Managing personnel, finance and operations of the Sheltered Home
• Documenting care activities in BumbleBee
• Submitting reports as required by funders
• Participating in team meetings and training

Qualifications:

• Diploma/Certificate preferably in social work/psychology/sociology/gerontology/counselling/nursing or the equivalent
• Minimum 1 year work experience in administration and community healthcare settings in Singapore
• Good communication and interpersonal skills (oral and written)
• Passionate in community services and service oriented
• Good judgement and decision making skills
• Good in problem solving
• Good record keeping abilities and attention to detail
• Meticulous and independent

Additional Advantegous Skills:

• Ability to speak English, Mandarin, Malay and Chinese dialects

Other Information:
Additional Responsibilities

• Conducting home visits and outreach activities to the community
• Other ad-hoc assignments at the discretion of program lead/ Department head
