Organise and co-conduct intake interviews with clients and caregivers
Collect information and conduct assessments of clients
Coordinate, implement and follow up on individual care plans for clients
Collaborate with Case Intervention Team to provide support and intervention to clients
Contribute to case reviews
Handle case related administrative duties and documentation
Adhere to the standard operating procedures and case work practices
Crisis Management Aspects:
Handle crisis situations, such as behavioural challenges in collaboration with relevant professionals
Mediate conflicts between clients, caregivers, and staff when necessary
Programme Management Aspects:
Work with team to design, implement, facilitate and review training programmes to equip clients with pre-vocational and vocational skills
Network and collaborate with relevant stakeholders and community partners
Maintain and compile statistics for reporting, funding or audit purposes
Qualifications:REQUIREMENTS
Diploma/Degree in Psychology, Counselling, Sociology or a related field
Knowledge and experience in working with special needs or disabilities will be an advantage
Good listening and communication skills to work with clients with diverse abilities and backgrounds
Good interpersonal skills to liaise with stakeholders and external agencies
Strong organisational and planning skills
Passionate and committed to working with people with disabilities
Good team player with a positive and resilient attitude
Proficient in Microsoft Office and administrative skills
Able to work on some weekends or weekday evenings
