Case ManagerChallengesYou will work within a multidisciplinary team to ensure continuity of holistic patient care, by assessing and evaluating needs of the patient, and planning, coordinating, implementing and monitoring the solutions to address those needs and to participate in other clinical care activities.As a Case Manager, you will be expected to:

  • Provide and coordinate continuity of inpatient and outpatient care from initial consultation through to recovery.
  • Act as pivotal team member for patient care by informing the multidisciplinary care management team of patient updates and initiate case review and discussion.
  • Provide holistic and comprehensive case management services to all patients during their inpatient stay and/or outpatient appointments, such as assessment of patients\' medical, psychological, social, environmental, vocational and financial needs, monitor and evaluate patient\'s response to treatment and revising treatment/care plan as needed etc.
  • Establish a thorough working knowledge of resources and information for dissemination to patient and family.
  • Establish rapport and build therapeutic alliance with patient and family.
  • Provide counselling and supportive therapy for patient and family in response to their needs.
  • Ensure proper documentation of patients within the case management system.
  • Perform any other assigned duty.
Requirements
  • Degree in Psychology, Social Work, or Nursing.
  • Working experience with individuals suffering from psychiatry disorder will be of added advantage.
  • Must be a team player with passion in patients\' care, willingness to learn, and able to contribute to the team.
