Working hours: 44hours / week Operation hours : 9am - 9pm (work 44hours per week follow schedule - included weekdays & weekend) Salary range: $ 2,500 - $3,500 Location: Buangkok, Singapore Job Responsibilities * Assess needs of seniors with care/s Working hours: 44hours / week Operation hours : 9am - 9pm (work 44hours per week follow schedule - included weekdays & weekend) Salary range: $ 2,500 - $3,500 Location: Buangkok, Singapore Job Responsibilities Assess needs of seniors with care/service needs and formulate care plans for seniors with care/service needs Assist in the development and implementation of program for seniors through data collection and the conduct of basic analysis Coordinate care for the seniors with relevant stakeholders Documentation of case notes Facilitate timely and appropriate service referral and provision to seniors Participate in case discussions with external service providers Provide case management services Perform needs assessment of seniors living in the community Prepare data for submission to relevant authorities Requirements Preferably a Degree in Social Work/Psychology Preferably experience in Social Service Sector or related field Proficient in MS Word, Excel and PowerPoint.
