Job Description
.
- Assess the psycho-social status of the patients and families / caregivers to determine the appropriate care plans
- Counsel and support the patients and families / caregivers associated with the illness
- Conduct home visits / care. When necessary, coordinate home modification for the patients
- Maintain working relationship with other disciplinary teams on patient care plans
- Work with other provider to provide appropriate community services to ensure continuity of care
- Share the sessions with students and other professionals
- Maintain the right documentation for case management and audit purposes
- Degree in Psychology/ Social Worker/Counselling/Gerontology
