In this connection, we are hiring talent to join our team in our HQ functions, social service support and direct social service. RESPONSIBILITIES The Case Worker is responsible for facilitating change and improving lives for individuals, families, community facing transitional challenges through the provision of social casework. 1. RESPONSIBILITIES / MAJOR ACTIVITIES Conduct casework management, including gathering information, conducting psycho-social assessment, identifying common needs and goals for intervention to r ender effective and professional social work services. For homeless clients, conduct assessment on accommodation matters, provide relevant assistance and referral to external agencies where necessary, collaborate with relevant stakeholders for referral to transitional shelters in order to explore reasons for homelessness as well as current accommodation and future housing needs and render necessary assistance such as employment, financial, housing options, etc. Provide basic counselling as required to support clients to identify goals and potential solutions to problems which cause emotional turmoil; improve communication and coping skills; strengthen self-esteem; promote behavioural change and optimal mental health. Plan, organise and implement community outreach activities, programmes and group work to help, uplift, and support those who are deprived of certain services and rights and promote active ageing and enhance quality of life for older adults. Perform evaluation and prepare reports for administrative investigation and intervention programmes through maintaining proper records of cases and providing regular and timely statistics of Clients and their progress. Provide information and referral services to link Clients to appropriate social services by collaborating with all stakeholders including primary care, secondary and acute medical care, community partners and external agencies to provide the best support to the clients and their families. Participate in research to contribute to evaluation of the efficacy of the practices in which social workers engage and the programmes they implement. 2.
