- Assist in the care management team to provide basic screening and measurements for chronic patients.
- Provide clinical support includes measuring patient\'s parameters, performing screening tests, diabetes screenings, and vaccination gaps.
- Coordinate, schedule and track appointments with Specialist Outpatient Clinics for patient referrals.
- Promote care through active engagement with patients to sign up for My Health Map.
- Min Nitec and above
- A team player with good interpersonal & communication skills
- Passionate to work in Healthcare industry
- Customer-oriented, a team player, a fast learner and serious with work
