About the Role

• Provide basic care coordination, discharge planning, monitor/evaluate and education for patients.

• Conduct telephone follow up to build rapport, obtain feedback and engage patients and their caregivers.

• Work closely with other community care coordinators and nurses to develop and manage care for the patients in the community.

• Support caregivers in caring for patients.

• Promote healthy lifestyle education, including educating the carers of the patients.

• Participate in community health lifestyle activities.

What We Are Looking For\xef\xbb\xbf

• Candidate must possess at least a Bachelor Degree

• Candidate should have at least 2 years of working experience as an Allied Health Personnel

• Good communication and interpersonal skills

• Strong organizational, coordination and team building skills

• Proficient in computer skills and Microsoft office

• Able to work independently and with a team

• May need to work on weekend for community activities
