Community Nurse

Job Description
As a Community Nurse, you will:
- Work closely with a team of dedicated healthcare professional to administer patient-centered care
- Visit patients in their home to provide nursing care, educate patients and their families
- Manage Community Nurse Post one-day per week on roster basis
- Update records of patients' progression after each home visit
- Hold a valid registration with Singapore Nursing Board.
- Possess a Diploma or Degree in Nursing educational qualification.
- Minimum 3 years of inpatient ward experience in local acute hospital preferred.
- Able to perform on-calls duty after office hours.
- Able to work on weekends when required.
