Work closely with team members in the following capacity relating to Social Prescribing:
1.Patient Engagement

Engage patients referred from the care team either in polyclinic or via telephone call on attending community activities.
Provide patients with a list of community activities, identify activities of interest and assist in sign-up.
Promote and encourage patients on the benefits of attending community activities.
Contact patient to follow-up on their attendance at the community activity.
Assist patient with way-finding and general enquiries.
Provide patients with a questionnaire and assist with completion, if required.
Community Partners Engagement

Consolidate community activities and its schedule from community partners in the vicinity of the polyclinic.
Collate patient sign-up sheet to community partners for tracking.
Obtain patient's attendance from community partners.
Administrative

Maintain a list of patients engaged and referred to community activities.
Update patient's care team on the community activity that patient is referred to.
SECONDARY RESPONSIBILITIES

Any other duties assigned by the supervisor, where appropriate.
