Job Purpose
To provide key support for the seamless coordination of patient care to enhance patient experience and care continuum.
To supervise, train and motivate junior staff, so as to achieve maximum patient care and satisfaction.
Duties and Responsibilities
Primary Duties and Responsibilities (80%)
Clinical assistant to clinicians in patients\' consultations, so as to provide seamless coordination of patient care, including (but not limited to):
Ensure that all necessary arrangements are made for patient\'s specialists consultations and investigations (blood test, pre-op workouts, radiology tests and etc.) and listing of surgeries
Provide patient counselling and education for surgical procedures, discharge care / treatment plans, wound care advice and etc.
Work with clinicians to develop clinical pathways / care plans for patients, including conducting audits on care plan and proposing actions for improvement
Work with Nurse Manager / deputies / Operation Manager to lead, mentor and support ancillary staff in service delivery and improvement (e.g. financial counselling, handling of feedback, email enquiries and communication to patients).
Adhere to department and hospital policies and procedures (eg: SOP)
Provide support in clinic operational areas
Secondary Duties and Responsibilities (20%)
Lead/support in quality improvement project (eg: 6S, RIE, service improvement project)
Perform and participate in hospital audit and review processes, e.g. Hand Hygiene, Environmental, Patients Record Review.
Assume any duties / responsibilities as assigned by Ops Head, Ops Manager and Clinic / Nurse Manager
Job Specification/ Requirements
Minimum degree with 2 years\' experience and above, preferably with healthcare experience.
Proficient in computer skills.
Team player with good communication and interpersonal skills.
Able to multitask and work in a fast pace environment.
