Key Position Objectives

Provide counselling and case management of clients of Club HEAL.

Duties & Responsibilities

To provide assessment, psychosocial interventions which includes case management and appropriate therapy(s) for clients with mental health issues and dementia, including their caregivers. Develop individualised intervention programmes (psychotherapy and counselling) Case manage and work closely with the multidisciplinary recovery/dementia team for each client and their caregivers (assess needs and develop care plans) Provide clients and their caregivers with education, training and support to equip and enable them to live well in the community Liaise with other service care providers of persons with Mental Health conditions/caregivers as needed Up-to-date reporting of each person with Mental Health conditions /caregiver To work closely with and utilise existing community mental health system and process for helping to coordinate patients to other forms of care if necessary Any other duties assigned by COMIT Manager

Preferred achievements / characteristics:

Bachelors or Masters in Counselling, Psychology or Social Work 2-3 years of relevant working experience in Social Service Sector or Mental Health Sector

Any other duties as assigned by Reporting Officer.
