Job Responsibilities:
Process information and statistics using an electronic system and gather information for financial assessments.
Conduct assessments and home visits when needed.
Liaise with agencies, authorities, and patient families.
Coordinate with nurses on patient-related matters.
Supervise customer care coordinators handling subsidy cases.
Act as a communication link between management and customer care coordinators.
Maintain and update patient records.
Stay updated on government policies regarding elderly care services and funding.
Job Requirements:
Minimum O Level
At least 1 year experience in healthcare industry
Minimum GCE 'O' Level / NITEC / Diploma or equivalent.
Prior experience in healthcare or customer service will be an advantage.
Team player with a service-oriented mindset.
