Hua Mei Elder-centred Programme of Integrated Comprehensive Care (EPICC) serves older persons who wish to continue living at home in spite of their complex medical conditions and limited family and so Hua Mei Elder-centred Programme of Integrated Comprehensive Care (EPICC) serves older persons who wish to continue living at home in spite of their complex medical conditions and limited family and social support. It provides team-managed, person-centred, integrated comprehensive care, with a day club programme. The EPICC team comprises of physician, nurse, social worker, physiotherapist, occupational therapist, centre manager, programme executive, programme assistants and driver. Reporting to the Programme Manager, the Enrolled Nurse provides clinical support and assistance to clients on a daily basis as an integral part of the operations of the centre. Key Responsibilities Provide basic nursing care and update on changes in clients' conditions and needs Assist RN in managing medications and in complying with safe medication practices Assist in simple clinical and rehabilitation activities Organize therapy as prescribed and maintain accurate documentation of care given Co-ordinate visits for medical check-ups and accompanies clients for follow-up appointments as needed Works with RN to ensure a safe and clean environment Participates in related projects, campaigns, training and development of care staff e.g. infection control Key Requirements WSQ Certificate in Healthcare Support (Nursing Care) or equivalent Minimum Nitec in Nursing registered with Singapore Nursing Board (Enrolled Nurse) Training/Experience in therapy aid, nursing aid is an advantage Able to work independently and as part of a multidisciplinary team Good working attitude Preferably with experience in eldercare from the social service sector Salary is indicative and will commensurate with experience and qualification.
