PRINCIPAL RESPONSIBILITIES

Evaluate, plan and implement care of patients with chronic kidney disease. Conduct teaching and counseling for patients and their families / carers. Coordinate patient care where required. Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services. Maintain related database. Provide continuous and relevant updates, and submission of reports to the Programme Manager and/or Renal Champion Doctor where required. Maintain a clean and safe working environment.

SECONDARY RESPONSIBILITIES

Assist to review and develop patient education materials in consultation with physicians or other health-care providers where required. Assist to review and evaluate work processes in consultation with physicians or other health-care providers where required. Precept newly joined coordinators in delivery of patient care where needed. Participate in quality improvement activities. Take on additional responsibilities or tasks as assigned by the Reporting Officers.

Requirements

Bachelor Degree in Nursing or Bachelor Degree in Life Science, Medical Sciences and/or related medical fields Proficient in Microsoft applications. * A team player with excellent interpersonal and communication skills.
