Key Responsibilities and Duties
- Engage, motivate and empower residents to lead healthier lifestyles, monitor their health status and delay frailty/chronic diseases;
- Organise and conduct population health programmes and activities including exercise sessions, health screenings, falls prevention programs, weight management programs, and health workshops/talks;
- Participate in design, development and implementation of programmes based on local community needs, strengths and assets
- Be the point-of-contact in the community to provide health coordination, navigation and communication for health initiatives
- Collaborate with community, social and primary care partners to coordinate care to meet the health needs of residents, including to address social determinants of health.
- Support documentation and tracking of data for programme monitoring and evaluation
- Participate in or support organisation/department projects as assigned
- A recognized Diploma, preferably in health-related fields like Sports Science, Nutrition, Psychology and Behavioural Science
- Prior frontline experience interacting with patients, residents or customers
- Health coaching experience will be an advantage but not compulsory as training will be provided
- Strong interest to work in the community to engage residents in improving their health
