Job Description


  • Adhere strictly to the workflow and standard operating procedures (SOPs) of the Health Screening and Follow-up Services Project
  • Coordinate with the Team Lead prior to the health screening / coaching session to facilitate a smooth and organized process
  • Prepare and organize coaching materials in advance to ensure a smooth and effective coaching session
  • Provide assistance to participants with sitting and standing, and identify individuals with special needs while verifying their identity with photo identification.
  • Assist participants with taking anthropometric (using measurement tape) and blood pressure measurements, including those with mobility challenges, frailty, or instability on the weighting scale.
  • Effectively communicate and interpret health screening results to participants, as well as collaboratively develop and establish S.M.A.R.T health goals that align with their individual needs and preferences.
  • Facilitate health coaching sessions by analyzing participants\' current and past health metrics, and reviewing their health screening reports and previous coaching notes to develop personalized coaching plans.
  • Health coaching also includes guiding the participants in physical activities.
  • Follow up with the participants for their next appointment.
  • Administer health and lifestyle survey as needed and record measurements and survey results immediately
  • Provide guidance to participants towards healthier lifestyle behaviors while maintaining accurate records of their progress and coaching notes.
  • Support in the dismantling of the site and packing up equipment and materials after health screening or coaching sessions, and transport equipment and forms back to clinic as instructed by Lead Health Coach or other health coaches.
  • Provide administrative support to the operations, including conducting roll calls and briefing sessions, timely and accurate communication, dissemination, and cascading of essential information to the team, following the schedule provided by HPB/PanCare.
  • Perform other duties as assigned by the Project Lead or Management.
Requirements
  • Be a Registered Staff Nurse with a valid practicing certificate from the Singapore Nursing Board (SNB)
  • Hold a valid First Aid/CPR certification and be proficient in using AED equipment
  • Provide first aid to participants who require medical attention
  • Possess knowledge of health promotion topics, particularly HPB messages, and platforms such as HPB\'s Healthy 365 app and Healthhub
  • Possess knowledge of basic laboratory procedures and equipment
  • Possess a professional demeanor and conduct themselves with integrity and respect towards all participants, staff, contractors, and partners
  • Demonstrate patience and approachability while assisting participants and handling challenging situations effectively
  • Demonstrate effective communication skills, both verbally and in writing, to interact with participants, staff, contractors, and partners
  • Display adaptability and flexibility to work efficiently in diverse settings with changing demands and schedules
  • Ability to follow infection control guidelines and protocols, and comply with local and national regulations related to blood collection and handling
