Job Summary:
\xc2\xb7 Support the expansion of St Luke\'s Hospital\'s (SLH) Western Silvercare programme (WSC) to new Community Health Posts (CHP) in the western region of Singapore.
\xc2\xb7 Support the Care Coordinator and Nurse in their efforts to promote preventive health in the community amongst seniors.
\xc2\xb7 Engage and connect seniors to WSC and other preventive health programmes in the community.
\xc2\xb7 Support the WSC team in the collection and management of participants\' and programme data.
Responsibilities and Duties:
Specific
\xc2\xb7 Assist the Western Silvercare team in coordinating, scheduling appointments, and referring seniors to preventive health programmes and services in the community.
\xc2\xb7 Perform basic preventive health screening and assessments at various CHP in the community.
\xc2\xb7 Provide basic health education to seniors.
\xc2\xb7 Document all care activities and assessments conducted at the CHPs into various IT and/or CRM systems.
Requirements:
\xc2\xb7 Qualifications in health management or health promotion preferred.
\xc2\xb7 Work experience in community care or healthcare sectors will be an advantage.
\xc2\xb7 Well-versed in Microsoft Office applications - Excel, Word, PowerPoint, with ability to navigate various IT and/or CRM systems.
\xc2\xb7 Keen interest in engaging seniors in the community.
\xc2\xb7 Strong interpersonal and relational skills.
\xc2\xb7 Self-motivated with good communication skills.
\xc2\xb7 Proactive and a good team player.
