JOB DESCRIPTION You will be the health buddy to the patients/residents, to navigate and link up with community resources available. You will also facilitate basic healthcare needs and arrangement between Specialist Outpatient Clinics and GPs and mo JOB DESCRIPTION You will be the health buddy to the patients/residents, to navigate and link up with community resources available. You will also facilitate basic healthcare needs and arrangement between Specialist Outpatient Clinics and GPs and monitor patients' follow-up and conduct home visits when necessary. You will also be an active agent in outreach and community engagement to promote healthy lifestyle. In addition, you will manage and maintain the Wellness and Care Centres and supported self-management activities. JOB REQUIREMENTS Minimum 'O'/'N' levels or equivalent NITEC in Community Care & Social Services or Certificate in Healthcare Assistant Course will be advantageous Passionate or experienced care-giver without minimum qualification may be considered Possess positive work and learning attitude and good interpersonal skills Physically fit and able to work independently Well versed in written and spoken English. Ability to communicate in local dialects/Mandarin during the house visits to the local dialect speaking elderly.
