Description
- This role will be abit more like case management - manage the care plan for the patients
- You will be the key person who will make the most difference as you make home visits to provide palliative care and recommend psycho-social support to our terminally ill patients and their families.
- You will be part of the most dedicated, compassionate and caring teams comprising doctors, nurses and medical social workers providing an all-in-one professional care to patients at home at all hours, seven days a week.
- Min Diploma/Degree in Nursing
- Advance Diploma in Geriatrics, Medical-Surgical, Chronic Disease Management, Community Health is advantageous
- Holds valid practicing certificate with Singapore Nursing Board
- Valid BCLS & AED certification/recertification
- At least 2 years\' experience nursing experience in Singapore
- Knowledge in community care is advantageous
