Plan and conduct seniors outreach activities to know our seniors in the assigned service boundary.
Conduct/support active ageing programmes suitable to our seniors and encourage active participation from seniors
Engage/Partner with our community partners and relevant government agencies to support their programmes for seniors
Promote NTUC Health services and programmes, from time to time. Where required, coordinate care and support services through internal referral or external agencies
Support any seniors outreach events and programmes that may happen after office hours and on weekends when needed
Carry out administrative functions such as data entry, record keeping and report submissions in a promptly and accurately
May require to perform counter duty and handle enquiries about promoting NTUC Health services and programmes.
Assist in care of seniors like, ambulation, feeding, toileting and transfers.
Assist in the care supervision of the elderly in the center with other care staff.
Assist in medication witness *(upon certification competent)
Assist in taking vital signs monitoring *(upon certification competent)
Assist in transport charpone *(upon certification competent)
Assist in co-facilitation of IGP programs with care staff
Assist in entry of documentation pertaining seniors into B3.
Any other duties as assigned.
