- Note: This is a non-patient fronting role. Mostly calling of patients.
- We are looking for Care Coordinators to proactively engage, educate and empower our patients to care for themselves!
- Care Coordinator works closely with doctors and nurses to promote healthy lifestyles and preventive health.
- Educate patients to understand the importance of preventive health such as foot, eye, health screenings and vaccinations.
- Screen patients for health conditions
- Provide tele-consult to patients on home self-monitoring, performing vision testing and basic assessments.
- Make recommendations to slot them in for other health appointments, if needed.
