Job Summary: The primary objective of the role is to help manage a patient's care while they're enrolled in the company's health program or health plan. He/She should ensure that the needs of his/her patients are being addressed. He/She educates patients on medical procedures or conditions and acts as a liaison between the patient and healthcare personnel by keeping communication lines open. Typically, the role focuses on answering patient's questions, managing patient's accounts and health journey and scheduling patient consultations/sessions with the health care professionals handling the patient.Qualifications:
