Job Description
Our client, a reputable healthcare company is looking for potential candidate to join their organization.
Responsibilities:
- Assess the psycho-social status of the patients and families / caregivers to determine the appropriate care plans
- Counsel and support the patients and families / caregivers associated with the illness
- Maintain working relationship with other disciplinary teams on patient care plans
- Work with other provider to provide appropriate community services to ensure continuity of care
- Share the sessions with students and other professionals
- Maintain the right documentation for case management and audit purposes
- Degree in Social Work
- Registered with Singapore Association of Social Work
- At least 2 years of related working experience
