Responsibilities

Interviews and counsels patients and their families to help them with their psycho-social problems.
Communicates with medical staff, nurses, other health-care professionals to discuss plans for total patient management.
Liaises with other government departments, social services organisations or private agencies to meet patients\' needs.
Maintains case records on patients.
Participates in the orientation of new staff.
Represents department on committees of statutory, voluntary and community organisation providing social services.
Attends supervision session, department meetings, case conference and training workshops and seminars.
Carries out psycho-social research and relevant special projects.
Visits patients\' homes to assess social conditions and family dynamics, counsels and gives support.

Requirements

Degree in Social Work or equivalent, preferably with Honours degree or Masters.
Preferably a Registered Social Worker (RSW) with Social Work Accreditation Board.
Prior experience in social work or the social agency setting is preferred.
Good counselling and communication skills.
Must be fully vaccinated with COVID-19 vaccinations.
