Roles & Responsibilities
Your job scope includes close collaborations with the multi-disciplinary team in clinical setting and recommend appropriate interventions to support/facilitate a holistic care plan for patients to successfully integrate back home and into the community.
You will conduct financial assessment, field visit, build good rapport with government / community / social agencies or individuals to mobilize social resources to meet patient\'s needs. You will also be expected to initiate and implement support programs / improvement projects / learning initiatives for the Medical Social Service Department.
- Degree or Postgraduate Diploma in Social Work is required
- At least 3 years of social work experience preferably in clinical setting
- Strong advocate for holistic and patient-centered approach to care of patients with medical condition(s) and/or disability
- Must be a team player who enjoys working in a fast paced environment
- Being a registered member with the Singapore Association of Social Workers
