Educates and supports residents as well as their families or caregivers to promote understanding and involvement in care.
Leads and coordinates discharge planning efforts for residents who are deemed suitable for discharge.
Ensures that nursing services are provided in line with recognized clinical standards and evidence-based practices.
Keeps nursing policies and standard operating procedures (SOPs) current to ensure consistent, high-quality care delivery.
Requirement :
More than 3 years of relevant working experience
