Work with the group of general practitioners (GPs) in the network to provide structured nurse counselling, health screening services, patient education on chronic disease, mental health management and health promotion through face-to-face or virtual consultations
Identify patients suitable for nurse counselling to help them understand their chronic conditions and improve their health outcomes
Assess patients' social history, current lifestyle and dietary habits to help patients bring about effective change and enhance their well-being
Work closely with the GPs in providing individualized care plans to patients to manage their chronic conditions effectively
Teach/advise patients on how to administer insulin injections, perform self-monitoring of glucose and home blood pressure readings, proper usage of inhalers, provision of targeted diet plans and other nurse-related counseling
Conduct eye and foot screening with patient education to diabetic patients and perform spirometry test to diagnose patients with asthma or COPD
Assess patients' mental well-being through questionnaires and escalating to the GPs when required.
Provide support to the admin team in tracking and monitoring of the clinical and outcome indicators for the clinics
Work closely with the Primary Care Coordinators (PCC) on scheduling patients for ancillary services
Support the PCN in quality improvement projects to achieve better clinical and process outcomes
Perform any other duties assigned by the Reporting Officer
Job Requirements
At least an Advanced Diploma in Nursing
Registered Nurse with the Singapore Nursing Board
Specialist Diploma in Diabetes Education/Gerontology or Advanced Diploma in Chronic Disease Management is preferred
Minimum 4-5 years of working experience, with experience in chronic disease management preferred
Experience in conducting health screening and provide comprehensive health education to patients with chronic conditions
Ability to provide good clinical judgment, possesses critical thinking and problem-solving skills in the delivery of patient care in line with established work instructions and protocols
Excellent communication and interpersonal skills are essential
Passionate about making a difference in improving the health of patients
Possess proactive attitude, team spirit and maturity
Meticulous and able to keep proper and timely documentation
Conversant in a second language/dialect would be advantage to speak with patients
Willing to travel to the northern and central parts of Singapore
Willing to work on Saturdays, when required.
