Comply with clinical protocols, service standards and accurate documentation in line with MOH service requirements; maintaining clinical and operational excellency.
Assess, Evaluate, Plan and Treat clients for optimal care.
Participate in Client Review meeting with other Allied Health team members.
Document and report adverse incidents to management, as necessary. Provide feedback on risk assessment and quality management activities.
Investigate and resolve all feedback and complaints received. Recommend and implement improvements.
Maintain a neat and tidy workplace, Adhere to infection control and prevention practices in premises.
Maintain and upkeep a proper inventory control with regular inspection and audits.
Observe changes in the clients\xe2\x80\x99 health conditions and report these to Centre Manager.
Establish and maintain effective communication with all other health professionals, support staff, families and care providers.
Assist in supporting staff in medical emergencies or crisis situations.
Teach clients, caregivers and support staff proper techniques in self-care.
Assist and support delivery of health promotion activities in the Centre.
Conduct physical and mental health talks as part of outreach/wellness events.
Keep abreast with advances in the profession and to contribute to the development of the department.
Participate in quality / clinical service improvement projects, infection control campaign activities, research and activities at departmental and organisational level and also relevant agencies.
Perform other duties as assigned by Centre Manager.
Qualifications:
Registered with The Singapore Nursing Board and possess a valid Practising Certificate.
Requirements:
Passion, willingness and patience in serving elderly clients.
Humble, willing to learn, a team player.
Self-driven, willing to take on challenges.
At least 2 years of professional practice experience; Experience working in dementia care, geriatrics or healthcare settings preferred.
Required to attend to elderly clients who are mostly mandarin/dialect-speaking. Able to speak Mandarin and/or dialects, besides English is essential.
