Job Description
Our Client, a leading and well-known Health Institution in Singapore is inviting qualified candidates to fill the position as Care Coordinator.
Candidates with relevant healthcare experiences are welcome to apply!
Summary:
- Salary up $2500 [Based on experiences and qualification]
- Various Location [Bukit Batok, Jurong, Bukit Panjang, Choa Chu Kang, Clementi, Queenstown, Pioneer]

- Assist in the care management team to provide basic screening and measurements for chronic patients.
- Educate patient on home monitoring devices
- Assist in planning, organizing and coordinating workplace health promotion activities for staff and perform any other duties as assigned by supervisors.
- Make appropriate referrals to care managers, assist patients to schedule follow-up appointments for consults, screenings and vaccinations in relation to chronic care management/population health.
- A team player with good interpersonal & communication skills
- Passionate to work in Healthcare industry
- Customer-oriented, a team player, a fast learner and serious with work
- Relevant experience in healthcare industry
