Pharmacist (ambulatory Care)

Job Description
Key responsibilities:
- Provide clinical pharmacy services in outpatient clinics, with focus on chronic disease management (e.g. diabetes, hypertension and anticoagulation) and medication optimisation
- Collaborate with multidisciplinary teams to enhance therapeutic outcomes and are transitions
- Participate in service development, quality improvement initiatives and patient education
- Full registration with Singapore Pharmacy Council
- Bachelor Degree / Masters in Pharmacy
- Good written and communication skill
- Strong foundation in chronic disease pharmacotherapy (e.g., diabetes, hypertension, anticoagulation)
- Skilled in therapeutic monitoring, medication reconciliation, and deprescribing
- Familiarity with evidence-based guidelines and ambulatory care protocols
- Engages in teaching, research, or professional advocacy
- Experience in collaborative care model will be an advantage
