Job Description
As part of the multi-disciplinary team with the National University Health System (NUHS)@Home, you will provide quality pharmaceutical care as our Pharmacist.
Job Responsibilities
- Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions
- Medication counselling on transfer medications by a variety of means (in-person and virtual)
- Medication reconciliation
- Review discharge prescriptions appropriately
- Verify new orders by the physicians and identify inappropriate medication/doses if relevant
- Assist with perform medication supply duty
- Follow up on medication preparation status
- Undertake duties which may be assigned by the NUHS staff member depending on the requirements
- Bachelor of Science in Pharmacy from an accredited college by Singapore Pharmacy Council
- At least 2 years of relevant experience
- Fully registered with the Singapore Pharmacy Council
- Possess excellent communication, interpersonal and organisation skills
- Able to work independently and in a team
- Undertake duties which may be assigned by the NUHS staff member depending on the requirement
- Weekdays and/or weekends working hours are available
