To provide and deliver safe and efficient pharmacy services in the hospital.
Key accountabilities
Review medication orders for completeness and appropriateness, and if needed, perform and document interventions.
Process drug orders and dispense medications to patients.
Counsel patients (inpatients / outpatients) on their medications and provide specialised counselling if needed (e.g. warfarin, direct oral anticoagulant, inhaler, and insulin injection counselling).
Perform medication reconciliation.
Provide drug information to doctors, nurses and patients.
Assist in the preparation of ad-hoc items e.g. extemporaneous preparations, reconstituted antibiotic suspensions.
Perform store / procurement duties (if assigned).
Perform medication-therapy related duties e.g. supply and receipt of controlled drugs, supply of drug stocks to patient care units, adverse drug reaction reporting, drug usage monitoring, etc.
Adhere to legal and hospital policy requirements.
Participate in quality assurance activities e.g. ward inspections/expiry checks (as assigned).
Collate records relating to drug supply, storage and distribution as required by hospital and legal regulations (i.e. JCI, MOH standards).
Participate in continuing professional education (CPE) programmes.
Assist in the training and supervision of pharmacy assistants and technicians.
Assist in providing clinical experience to pre-registration pharmacists and students on attachment (if applicable).
Perform extension hours, night duties and/or on-call duties (as assigned).
Qualifications & Experience
Bachelor of Science in Pharmacy from an accredited college of pharmacy or university
Registered with Singapore Pharmacy Council
Preferably with working experience in hospital setting.
