As a regional provider for integrated healthcare servcies in Singapore, Malaysia and China, the Group incorporates a chain of medicare centres and nursing homes, hospital services, a careskill training centre, a chain of senior services centres, home care services, ambulance services, physiotherapy services and traditional Chinese medicine services. The Group also provides consultancy for the development and management of retirement living communities. RESPONSIBILITIES

  • Plan and conduct on-site activities for seniors that promotes social interaction, and health and wellness in a group setting
  • Develop and implement annual calendar plan of social and health activities for seniors
  • Ensure that the programme and activities meet the performance indicators and operating guidelines issues by MOH
  • To oversee the day-to-day operation and provide supervision to staff and volunteers
  • Initiate a case record file for every senior household in the identified block and capture the needs of the seniors
  • Establish and motivate seniors to run mutual help groups; senior volunteers
  • Network with other agencies to establish additional avenue of help for the welfare of the seniors
  • Establish networks with local community-based services or resources to match the need of the seniors
  • Oversee and manage the Alert Alarm System (AAS) during the operating hours, rendering assistance when necessary
  • Maintain an updated register of seniors, volunteers and any other records critical for follow-up and reporting to organisation and relevant authorities
  • Conduct home care assessments and complete initial assessments (using Community Screening Tool) of senior and family to determine home care needs
  • Plan and conduct on-site activates for seniors that promotes social interaction, and health and wellness in a group setting
  • Supervise and assist seniors when participating in the activities
  • Ensure that the centre facilities and premises are in clean and good condition and maintenance is regularly executed
  • Ensure the centre facilities and premises safety and hygiene measures are in place and are properly executed
  • Perform such other duties and responsibilities that may be assigned from time to time
Requirements
  • Advance/Higher/Certificate in Community & Social Service (Senor Care Services) and good “O” level (minimum 3 credits)
  • Competency in eldercare service with at least 2 years working experience in an eldercare institution
  • Ability to understand and speak local dialects will be an advantageous
  • Able to perform administrative tasks and possess basic Microsoft Office literacy
  • Able to prepare and conduct care programmes for seniors; e.g daily news chats/cooking therapy/art & craft/singing sessions/dancing, reminiscence therapy; etc.
  • Self-motivated team player who enjoys working with different profiles of seniors
QUALIFICATIONS
