Job Description


:

The Programme Executive shall work closely with Centre Supervisor and team, community partners and volunteers to deliver the ABCs for Active Ageing Centre service model. Through the progammes and services, seniors can be engaged meaningfully to help them age well in place.

  • Be part of a team in running the operations and delivering programmes and services of an Active Ageing Centre \xe2\x80\x93 Active Ageing Programme, Befriending and Care & Support.
  • Conduct assessments using Community Screening Tool, plan outreach/centre programs, provide home visitations for seniors.
  • Attend to enquiries and provide information and referrals to relevant agencies.
  • Motivate seniors to participate in Centre\xe2\x80\x99s programmes to maintain good physical, cognitive, emotional, social and spiritual health
  • Cultivate strong relationships with community stakeholders, corporate partners and volunteers to help the AAC thrive in the community.
  • Manage administrative duties and assist Centre Supervisor to prepare KPI submissions to MOH.
  • Be part of team to ensure the Active Ageing Centre operates with good governance and stewardship.
Requirements:
  • ITE/Diploma in relevant disciplines such as Community Care and Social Service, Gerontology, or other relevant fields in Social Sciences
  • 2-3 years of relevant working experience, preferably in a SAC or AAC or have volunteered previously with seniors in a community setting
  • Excellent interpersonal and communication skills, especially with seniors
  • Proficient in IT
  • Able to converse with elderly in English, Mandarin and/or dialects (in order to handle queries and concerns from English, Mandarin and/or dialects speaking patient and/or clients)
  • Resourceful, proactive, responsible and client-centric
  • Ability to multi-task and meet deadlines in a fast-paced environment
  • Team player with strong planning and organising skills
Qualifications:
  • ITE/Diploma in relevant disciplines such as Community Care and Social Service, Gerontology, or other relevant fields in Social Sciences
