• Centre Based Services including HNF and Outsource Centre(s):

• Perform assessment of clients for suitability of admissions.
• Perform assessment, care planning and outcome evaluation of the client on admission, every 6 monthly and whenever the patient\'s condition changes.
• Plan, implement and evaluate nursing procedures / management.
• Ensure safe administration of medications, and timely and accurate documentation including supervision of nursing aide during the administration process.
• Report any medication error or adverse drug event to Centre Manager and raise report, if required.
• Supervise the care staff to maintain a safe environment and practices as well as adhere to infection control practices.
• Educate and train patient and caregivers with the required skills and knowledge to achieve independence and self-reliance as much as possible.
• Ensure clinical related documentation are updated in a timely manner.
• Develop discharge plan and refer patient to other service provider to transfer care if necessary.
• Escalate care to the Nurse Clinician in an appropriate and timely manner.
• Perform assessment and render first aid measures when there is injury incurred.
• Initiate and lead resuscitation or first aid procedures for various clinical emergencies.
• Assist Centre Manager in reviewing of client\'s clinical condition and suitability for programme(s) in the centre such as outings and social activities.
• Act as a resource for clinical related issues to the Centre Manager.

• Conduct Caregiver Training
• Coverage of home care services as assigned, including cross coverage of teams
• Other Outsource service(s) e.g. home bathing service as assigned.

Job Requirements

• Registered Nurse with the Singapore Nursing Board
• Minimum 3 years\' experience working in working in acute hospital setting and/or community hospital setting.
• Possess Diploma in Nursing.
• Possess good communication skills to establish therapeutic relationship that fosters optimal quality patient care.

Working Hours

Monday to Thursday: 8am to 5.30pm

Friday: 8am to 5pm
