Screen referral based onpatients needs, preferences and care goals with consideration of resourceavailability and efficiency.
Analyse biopsychosocialassessment findings for patients to identify complications and evaluatefollow-up actions.
Implement care plans andcarry out the care interventions based on the assessment findings of thedoctor.
Perform timely andappropriate follow up including updating the service plan for effectivescheduling.
Collaborate withmultidisciplinary team for care integration and transition
Coordinate care to deliver holistic patient care with all members ofthe care team
Handle incoming clinical enquiries via various modes of communication.
Perform clinical triage for stable patients requesting for adhocconsultation.
Escalate to Supervisor when it is beyond her capacity to handle
Assist in managingunexpected and abnormal changes of clients health and social conditions andcollaborate with care team to resolve the issues.
Escalate to Assistant Nurse Clinician and/orCare team on complex clinical enquires and any problemsencountered in patients condition and/or in the handling of patients and/orcare-givers/family members.
Buildrapport therapeutic relationships and empower the clients, families and/orcaregivers for self- management through health promotion, caregivers trainingand motivational techniques
Liaise with patients/NOK,pharmacist and physician on prescription-related issues.
Inform operation team on laboratory orders and any special instructionto facilitate scheduling with outsourced laboratory.
Ensure that investigation forms are duly signed by doctor.
Receive critical laboratoryresult and provide timely communication with the primary doctor.
Perform read-back of thelaboratory results and carry out follow-up action with the patient, ifrequired.
Document the details of thecommunication and doctors order accurately.
Performing Home visits or telephonic care assessments when necessary tosupport holistic patient care e.g. SMF
Maintaintimely and accurate documentation of patients condition and care given
Engage patients, family andcaregivers for advance care planning in collaboration with the care teams.
Advocate for patients,families and caregivers on community needs based on their readiness.
Perform Swab for patients when activated
Perform Caregiverstraining/health promotion programme for clients, families and/or caregiversaccording to their learning styles and readiness.
Participate in Inservice Education and peer sharing sessions(Medication, wound rounding).

Job Requirements
Registered Nurse with the Singapore NursingBoard.
Minimum 5 yearsexperience working in acute hospital setting and/or community hospital/ caresetting.
Degree in Nursing and/or Advanced Gerontologyor other specialty preferred; experience inHome Nursing (preferred).
Possess good communicationskills to establish therapeutic relationship that fosters optimal qualitypatient care.
Has good telephoneetiquette.
Ableto make clinical judgements regarding patient care with minimal guidance.
Salary range: $2300 - $6000
Working location: Toa Payoh
Working hours: Monday to Thursday: 8.00am to 5.30pm,Friday: 8.00am to 5.00pm
