Launched in May 2024, WH@Home is an innovative "virtual" ward pilot program that enables eligible patients to receive the same high standard of hospital care from the comfort of their own homes, complemented by 24/7 access to a dedicated clinical team. This initiative is part of the national MIC@Home project, which aims to alleviate the pressure on acute hospital beds by assessing the feasibility of providing care at home for patients who do not require high-acuity monitoring. The program emphasizes three key aspects: a) Simple Care: Focusing on the needs that matter most to patients and their next of kin. b) Safe Care: Providing care with a calculated risk approach while ensuring a robust safety net. c) Scalable Care: Achieving cost-efficiency and ease of scalability without compromising the quality of care or resources. The Senior / Assistant Manager will work closely with Medical, Nursing, Allied Health and Ops Departments across Woodlands Health to advance and sustain the WH@Home programme. This position involves supporting the Manager, Senior Manager, or Deputy Director in guiding operational teams and spearheading project initiatives.
MAJOR DUTIES & RESPONSIBILITIES

Main Work Streams
Programme design, implementation, and evaluation
Develop clinical and operational pathways that advance and sustain WH@Home.
These include:
Model of care development
New referral sources
New care paths
Plan and innovate clinical operations to implement new initiatives
Identify and implement innovative digital health solutions and system integration solutions to enhance programme delivery and patient outcomes
Infrastructure, manpower, analytics, technology and other requirements needed to implement the programme
Data collection, monitoring and analysis
Perform data collection, data analysis and data management for BAU reporting
Coordinate the data efforts required for reporting to WH Management, NHG, MOH/MOHT, and other relevant stakeholders
To liaise with relevant stakeholders for data report submission
Procurement
Administrative and secretariat support for purchase related activities for medical and non- medical equipment including hospital wide standardisation of equipment, integration between equipment and workflows, as well as management of funds
Writing of proposal papers to facilitate approval of various stages such as pre-sourcing, evaluation and award
Management of timelines to ensure timely follow-up and closure of key action items
Strategic Development & Implementation
Support the strategic planning and development effort for WH@Home.
Foster partnership with key stakeholders including internal leaders from other enterprise, Clinical Leaderships, NHG HQ as well as external partners (community partners and other PHIs)
Secretariat support for committee and workgroup meetings
Plan and innovate programmes to future-proof WH@Home with emphasis on financial and process efficiencies
Serve as the operational coverage for cluster-wide activities during management absence, ensuring continuity of critical functions and decision making.
Support cluster-level strategic initiatives and facilitate cross-departmental collaboration to enhance integrated care delivery
General * Support the annual division work plan and budgeting exercise
Be the secretariat for relevant steering committees and workgroups
Carry out any other duties as may be reasonably be required by the Head of Department
Weekend work might be required.
JOB REQUIREMENTS Education / Training / Experience * University Degree
Well-versed in Microsoft Office applications - Excel, Word, Power Point
At least 3-4 years of related experience in a similar capacity, preferably in an operations or service background
Healthcare or community care experience is a bonus
