Care@Community Department aims to deliver holistic care that is centred on an individual's needs. It mainly targets seniors that are frail, whom have multiple care needs, that will become increasing complex with time. With the integration of community-based aged and social care services, it enables the seniors to continue to stay in the community for as long as possible, therefore allowing them to age-in-place.
Role

The Admin POC is a dedicated resource to support the organisation with working through all the relevant aspects of the partnership model used to operate the ICCP in the sub-region.
The Admin POC supports the partner organisations in the sub-region to build strong partnerships, align goals, establish roles and practice collaborating and working as a "team" to achieve the ICCP objectives.
Responsibilities
:
Secretariat and Coordination Function
Work with leadership team of the partner organisations to align goals, establish roles, and develop strategies and workplan for the sub-region to achieve the deliverables specified for Phase 1 and 2.
Work with centre managers and ground teams to implement the sub-regional workplan, and monitor and report on the progress of the workplan.
Organise and facilitate regular meetings and check-ins for centre managers and ground staff to build understanding and work through implementation and other challenges.
Engage and build networks and partnerships with other community care providers, regional health systems, grassroot organisations and other stakeholders.
Work with ICCP providers to trial joint referral workflow in Phase 1 to support care coordination deliverables in Phase 2.
Work with the AIC's sub-regional Relationship Manager on the reporting and monitoring of indicators.
Administrative Function
Collect and consolidate the information required for the quarterly progress report
Represent the sub-region to attend meetings convened by AIC as required, and report back to the leadership team of the partner organisations
Ensure that the endorsed report and claim form are submitted to AIC in a timely manner
Requirements:
Diploma Holder and above
A team player
Strong communication & interpersonal skills
Experience in coordination work
Meticulous & organizational skills
Possess creative-thinking and problem-solving skills
Able to multi-task & work independently with minimum supervision
