Healthier SGMission : Coordinate and implement Healthier SG programmes, working closely with MOH divisions,Clusters, PCN/GPs, WOG and community partners.
Primarycare plays an important role as the foundation of our healthcare system in supporting population health outcomes from cradle to grave. It enables good quality, comprehensive, affordable and continued care for Singaporeans near their homes.
To improve population health outcomes, MOH will be embarking on a major initiative, Healthier SG, to transform and elevate the importance of primary care and our family physicians. We will be taking firm steps to organise and resource our healthcare clusters to work with and support family physicians to keep our population healthy. We will also roll out a National Healthier SG enrolment programme, where the population will be encouraged to choose a family physician of their choice, as their first line of care.
The Healthier SG Implementation (HSI) main workgroup is set up to co-create the implementation of Healthier SG. The HSI main workgroup will work closely with GP leaders, members of professional bodies and cluster representatives to co-create the Healthier SG enrolment programme for the baseline launch targeted around June 2023. Supporting the HSI main workgroup will be 4 sub-workgroups, including (i) CareProtocols – Preventive/Chronic; (ii) Subsidised drugs; (iii) Primary Care IT; and (iv) Role of PCNs. Each sub-workgroup will deep dive into the respective domain areas, and work towards recommending viable solutions for implementation.
For this position, this individual will be part of a joint secretariat team between MOH and MOHT to develop Care Protocols to guide participation of primary care practitioners inHealthier SG. It is a dynamic, fast-paced job that requires working closely with policy teams as well as General Practitioners who are leaders within their sector.
