Conduct comprehensive and advanced bio-psycho-social-spiritual assessments for residents, especially those with complex or high-risk care needs.
Manage a caseload of complex cases through systemic assessment, individualized care planning, intervention, and ongoing review.
Provide individual and family counselling to support adjustment to institutional living, manage grief and loss, and cope with illness, disability, and end-of-life issues, including facilitation of Advance Care Planning (ACP).
Lead the assessment and facilitation of applications for national financial schemes and subsidies (e.g., Eldercare, Medifund).
Coordinate and oversee discharge planning, ensuring appropriate community linkages and support systems are in place for residents' safe transition to home or other care settings.
2. Clinical Supervision and Professional Development
Provide formal clinical supervision, mentorship, and professional guidance to Social Workers and Social Work Associates
Review and endorse assessment reports, case documentation, and intervention plans to ensure quality and adherence to professional standards.
Contribute to staff development through training, reflective practice, and knowledge sharing.
3. Program Leadership
Lead the design, implementation, and evaluation of psychosocial programs (e.g., group work, support groups) that enhance residents' emotional, mental, and social well-being.
Utilize outcome-based approaches to assess program effectiveness and recommend improvements.
4. Multidisciplinary Collaboration and Advocacy
Actively participate in and/or lead interdisciplinary care conferences, ensuring psychosocial considerations are integrated into residents' care plans.
Build and maintain strong collaborative relationships with internal departments, community partners, healthcare institutions, and government agencies (e.g., AIC, NCSS).
Serve as a key advocate for residents, representing their interests and rights in complex or ethically challenging situations.
Job Requirements:
Minimum Degree in Social Work.
Minimum of 5 years of relevant experience, preferably in geriatrics, elderly care, or nursing home settings.
Ability to converse in local languages so as to be able to communicate with residents and next-of-kin.
Keen to work in eldercare sector.
Team player with good interpersonal, organisational and communication skills.
Accredited with Singapore Association of Social Workers (SASW).
